มาตรการการเข้าจังหวัดภูเก็ต (เผยแพร่ วันที่ 19 ก.ค. 2564)

จังหวัดภูเก็ตยกระดับมาตรการการตรวจคัดกรองการเดินทางเข้าจังหวัดภูเก็ตสำหรับคนไทยจากต่างจังหวัดหรือคนต่างชาติที่อาศัยอยู่ในประเทศไทย คำสั่งจังหวัดภูเก็ตที่ 4021/2564

📅 มีผล 20 ก.ค. – 2 ส.ค.64

🔴🔴🟠 ผู้ที่เดินทางจาก 76 จังหวัด ที่ต้องการเข้าจังหวัดภูเก็ตต้อง (ข้อ 1 หรือ ข้อ 2 + ข้อ 3)
📌 ข้อ 1 ได้รับวัคซีนป้องกันโรคโควิด – 19 ตามที่จังหวัดกำหนดคือ ซิโนแวค (Sinovac), ซิโนฟาร์ม (Sinopharm) ต้องครบ 2 เข็ม ส่วน แอสตราเซนิกา (AstraZeneca), ไฟเซอร์ (Pfizer), โมเดอร์นา (Moderna), จอห์นสัน แอนด์จอห์นสัน (Johnson and Johnson) จำนวน 1 เข็ม อย่างน้อย 14 วัน
หรือ
📌 ข้อ 2 เป็นผู้ที่หายจากอาการป่วยด้วยโรคโควิด – 19 มาแล้วไม่เกิน 90 วัน
และ
📌 ข้อ 3 ต้องได้รับการตรวจหาเชื้อโควิด – 19 ด้วยวิธีการ RT- PCR หรือวิธีการ Antigen Test ไม่เกิน 7 วัน (ตรวจเองไม่ได้ต้องตรวจกับสถานพยาบาลและมีเอกสารรับรองการตรวจเท่านั้น)

🏠 คนในจังหวัดภูเก็ต ที่ต้องการเข้าจังหวัดภูเก็ตต้อง แสดงหลักฐาน ดังนี้

🏠 บัตรประชาชนภูเก็ต (ข้อ 1 หรือ ข้อ 2 หรือ ข้อ 3)
(ข้อ 1) ได้รับวัคซีนป้องกันโรคโควิด – 19 ตามที่จังหวัดกำหนด หรือ (ข้อ 2) เป็นผู้ที่หายจากอาการป่วยด้วยโรคโควิด – 19 มาแล้วไม่เกิน 90 วัน หรือ (ข้อ 3) ต้องได้รับการตรวจหาเชื้อโควิด – 19 ตามที่จังหวัดกำหนด

🏠 บัตรประชาชนต่างจังหวัด ภูมิลำเนาในภูเก็ต แต่ไม่มีชื่อในทะเบียนบ้าน อาศัยอยู่ในภูเก็ต (ข้อ 1 หรือ ข้อ 2 หรือ ข้อ 3 + หนังสือรับรอง)
(ข้อ 1) ได้รับวัคซีนป้องกันโรคโควิด – 19 ตามที่จังหวัดกำหนด หรือ (ข้อ 2) เป็นผู้ที่หายจากอาการป่วยด้วยโรคโควิด – 19 มาแล้วไม่เกิน 90 วัน หรือ (ข้อ 3) ต้องได้รับการตรวจหาเชื้อโควิด – 19 ตามที่จังหวัดกำหนด และ มีหนังสือรับรองจากพนักงานฝ่ายปกครอง นายอำเภอ หรือผู้ได้รับมอบหมาย

** เด็กต่ำกว่า 6 ขวบมากับผู้ปกครองได้รับการยกเว้นไม่ต้องตรวจ เด็กอายุระหว่าง 6-18 ยังไม่สามารถฉีดวัคซีนได้ต้องมีผลตรวจหาเชื้อโควิด – 19 ก่อนเข้า อายุมากกว่า 18 ต้องทำตามมาตรการเหมือนผู้ใหญ่
*** นักเรียนนักศึกษาอายุไม่ถึง 18 ปี ที่ไม่สามารถฉีดวัคซีนได้และต้องผ่านเข้า – ออกประจำเพื่อการเรียน ให้ติดต่อสถานศึกษาออกบัตรประจำตัวแสดงต่อเจ้าหน้าที่เวลาผ่านเข้า-ออกและให้ทางสสจ.ตรวจหาเชื้อและออกใบรับรองมีผลใช้ได้ครั้งละไม่เกิน 1 เดือน

✅ ข้อ 4 ต้องดาวน์โหลด “หมอชนะ” และยินยอมเปิดแชร์ตำแหน่งที่ตั้ง (Location)
✅ ข้อ 5 แสดงเอกสารหลักฐานข้างต้น ต่อเจ้าพนักงานควบคุมโรคติดต่อก่อนเข้าจังหวัดภูเก็ต
✅ ข้อ 6 การใช้เส้นทางคมนาคมและการตรวจคัดกรองการเดินทาง ผ่านทางด่านตรวจท่าฉัตรไชย ห้ามบุคคลและยานพาหนะทุกประเภทเข้าจังหวัดภูเก็ต ระหว่างเวลา 23.00 – 04.00น. ของวันรุ่งขึ้น
✅ ข้อ 7 ให้สังเกตติดตามอาการตนเอง (Self Monitoring) ตามมาตรการป้องกันควบคุมโรคหากพบอาการป่วยหรือสงสัยว่ามีอาการป่วยด้วยโรคโควิด – 19 ให้พบแพทย์โดยด่วน

Summary of Phuket Provincial Order No. 4021/2564


Phuket Province has upgraded the travel screening measures arriving into Phuket for Thais AND foreigners living in Thailand alike. With effect from 20th July 2021 until 2nd August 2021***
.
A. Those traveling from 76 provinces who wish to enter Phuket MUST be able to meet these requirements (item 1 OR item 2 AND item 3):
1 – Received COVID-19 vaccine as follows;
2 doses of Sinovac, Sinopharm or
1 dose of either AstraZeneca, Pfizer, Moderna or Johnson & Johnson at least 14 days before arriving Phuket

OR
.
2 – Fully recovered from Covid-19 for not more than 90 days.
.
AND 3 – MUST ALSO provide a negative test result for COVID-19 by RT-PCR OR Rapid Antigen Test not more than 7 days before travelling (must be tested with a medical facility and have a certificate of examination only)
.
B. Phuket Residents ID card MUST provide 1 or 2 OR 3 below:
1 – Received COVID-19 vaccine namely
2 doses of Sinovac, Sinopharm or
1 dose of AstraZeneca, Pfizer, Moderna or Johnson & Johnson at least 14 days before arriving in Phuket

OR
.
2 – Fully recovered from COVID-19 for not more than 90 days.
.
AND MUST ALSO
3 – Provide negative tested for COVID-19 by RT-PCR or Rapid Antigen Test not more than 7- days (must be tested at a medical facility and have a certificate of examination)
.
C. Other Provincial ID card holder who reside in Phuket but do not have their names registered in the household registration book (Tabien Bann) as living in Phuket provide item 1 OR 2 OR 3 ALSO a certificate from their District Administrative Officer (Nai Amphur) or other Government official.
.
1 – Received COVID-19 vaccine namely
2 doses of Sinovac, Sinopharm or
1 dose of AstraZeneca, Pfizer, Moderna or Johnson & Johnson at least 14 days before arriving in Phuket
OR
.
2 – Fully recovered from COVID-19 for not more than 90 days.
.
AND MUST ALSO
3 – Provide negative tested for COVID-19 by RT-PCR or Antigen Test method for NOT more than 7 days (must be tested with a medical facility and have a certificate of examination only)
.
** Children under 6 years old accompanied by vaccinated parents are exempt from examination.
Unvaccinated children between the ages of 6-18, must have a negative result for COVID-19 before entering Phuket.
Over 18 years old must follow the same measures as adults mentioned above.
.
*** Students under 18 years old unvaccinated and have to travel to Phuket regularly to study MUST contact the educational institution to issue an identification card to provide to health officer when entering and exiting Phuket and also need to have the Covid19 test certificate from the Phuket health administrative (valid for not more than 1 month each time).

Article 4 Must download “Mor Chana” application and agree to turn on location sharing function
Article 5 presents the above documents to the Communicable Disease Control Officer before entering Phuket
Article 6 ALL types of land transport passing through the Chatchai checkpoint are NOT allowed to enter Phuket between 23.00 hrs. – 04.00 hrs.

Article 7 To ‘self-monitoring’ according to ‘Preventative Measures to Control the Disease’ and if symptoms of COVID-19 are found or suspected contact a doctor immediately.
